• Provides direct care to those with intellectual and developmental disabilities.
• Overseeing residents and taking care of their needs, and performing housekeeping duties. Includes planning meals, shopping for groceries, preparing and serving food/meals, feeding and clean-up.
• Providing transportation to and from activities and appointments.
• Assisting clients with administration of medications (with proper training).
• Performs personal care tasks, including assistance with basic personal hygiene and grooming, feeding, and ambulation, medical monitoring, and health care related tasks.
• Facilitate resident’s inclusion in the community's experiences and activities.
  Requirements:
• High school diploma, GED, or equivalent
• 18 years of age or older
• Valid driver’s license, own reliable vehicle and good driving record
  

Locations: Brooklyn Park (Knox House)

• Plan, organize, coordinate and provides skilled nursing care to home care patients according to a plan of care, including advanced nursing interventions related to the patients' needs. 
• Maintain an individualized plan of care, related to the patient's needs, in collaboration with the physician, patient and significant others. 
• Make recommendations for devices or tools that might improve the patient’s quality of life.
• Administer medication as prescribed by the patient’s Physician.
  Requirements:
• Must be licensed as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Minnesota
• Must demonstrate the ability to read, write and carry out written as well as verbal directions in English 
• Must be able to lift, turn, position and transfer up to 50lbs

• Under the supervision of the Program Director, oversee the day-to-day operation of the program.
• Provide coaching to Direct Support Professionals and supervise daily activities of Direct Support Professionals.
• Coordinate the activities for, and maintain schedules of, the employees of the program.
• Provide input and feedback in hiring, disciplinary, and firing decisions.
• Responsible for ensuring that on-boarding, orientation and ongoing training is delivered.
• Advancing residents’ decision-making, productivity and participation in a range of activities.

            Requirements:

• High school diploma, GED, or equivalent
• 18 years of age or older
• Valid driver’s license, own reliable vehicle and good driving record
• Relevant experience
